This Notice describes how protected health information (“PHI”) about you may be used and disclosed, and how you can access this information. Please review it carefully.
Our Pledge Regarding Your Health Information
Renewed Wellness with Kelly LLC understands that information about your health is personal, and we are committed to protecting it. This Notice applies to records generated in connection with your nutrition coaching and/or medical nutrition therapy (MNT) services with us, whether created by Kelly or, where applicable, under the clinical supervision of Katie Garrott at VivaWise Functional Wellness.
We are required by law to:
- Keep your PHI private
- Provide you with this Notice describing our practices
- Follow the terms currently in effect
- Notify you if a breach compromises your PHI
We reserve the right to change this Notice and to apply the revised terms to PHI we already maintain. Any updated Notice will be posted on this page.
A Note on Scope
Renewed Wellness with Kelly LLC provides nutrition education and coaching. Medical nutrition therapy services are provided under the direct clinical supervision of Katie Garrott, CNS, at VivaWise Functional Wellness.
How We May Use and Disclose Your Health Information
For Your Care. We may use and disclose your PHI to provide, coordinate, and manage your nutrition coaching or MNT services — for example, reviewing your intake information, tracking your progress, or coordinating with your supervising clinician.
For Payment. We may use and disclose PHI so that services you receive may be billed to and paid by you or, where applicable, a third party.
For Health Care Operations. We may use PHI for quality review, case coordination, and other activities necessary to run our practice responsibly.
With Your Supervising Clinician. Where services are provided under supervision, we may share relevant PHI with Katie Garrott/VivaWise for training, consultation, or case review purposes connected to your care.
As Required by Law. We will disclose PHI when required to do so by federal, state, or local law.
To Prevent a Serious Threat. We may disclose PHI when necessary to prevent a serious threat to your health and safety or that of others.
Business Associates. We may share PHI with service providers who perform functions on our behalf (e.g., our scheduling and records platform, Healthie), and we require them to appropriately safeguard your information.
Other Uses. For any purpose not described above, we will obtain your written authorization before using or disclosing your PHI. You may revoke this authorization in writing at any time, except to the extent we have already relied on it.
Your Rights Regarding Your Health Information
You have the right to:
- Inspect and copy your PHI, by submitting a written request. We will respond within 30 days and may charge a reasonable fee for copies.
- Request an amendment to PHI you believe is incorrect or incomplete, in writing, with your reasoning. We will respond within 60 days.
- Request an accounting of disclosures we have made of your PHI (with certain exceptions, such as disclosures for treatment, payment, or operations).
- Request restrictions on how we use or disclose your PHI for treatment, payment, or operations. We are not required to agree, but will comply with any restriction we accept.
- Request confidential communications — for example, that we contact you only at a certain phone number or email.
- Receive a paper copy of this Notice at any time, upon request.

How to File a Complaint
If you believe your privacy rights have been violated, you may file a complaint with us directly, or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not take any retaliatory action against you for filing a complaint.
Changes to This Notice
We may update this Notice from time to time. Any revised Notice will be posted on this page with an updated “Last updated” date.
